Parents of incoming K students, please make sure you have submitted your child's most recent physical form with up to date immunizations before the first day of school. This may also be faxed to 781-659-8812.

 

Kindergarten Students will need:

 

5 doses DTaP

3 doses Hepatitis B

4 doses Polio

2 doses MMR

2 Doses Varicella Vaccine or Physician documented Varicella Disease 

 

 

 

 Students in Grade 4 will need to have an up to date Physical Exam submitted this school year. Make sure to schedule an appointment with your child's pediatrician if necessary.
